CREWKERNE Pharmacy, in Crewkerne Healthcare Centre, is taking part in the National Association of Primary Care dementia signposting pilot scheme.

The pharmacy will be a ‘Primary Care Navigator’ for patients diagnosed with dementia and their carer.

Once a diagnosis has been made by the GP they will be given a contact at Crewkerne Pharmacy who they can get in touch with for further information and support.
